Yeah, actually, a double-charged 9mm is REALLY obvious- it overflows the cartridge. Now, a .45 ACP, THOSE you can double-charge (if you're using 230 grain bullets) and not overflow the cartridge. As to 120mm... I don't really know what kind of loading density they're at, but I suspect it's near capacity. I do know they use a MIGHTY big powder grain in tank and artillery ammunition, though. As in, I wouldn't really call it POWDER, each grain is the size of a AA battery!

Ive only seen that happen one time in real life. the crew had done drills in a fighting position and apparently got dirt packed into the muzzle. They moved onto the range and boom, they banana peeled the barrel, just like that.

I saw a gun tube like that once; was told that the crew forgot to remove the boresight mirror before going wet.
